MindMatters Plus GP Initiative
Fifteen Divisions of General Practice worked with 15 of the MindMatters Plus schools. Sound partnerships were forged with outcomes such as clinical pathways guidelines, lists of local general practitioners (GPs) and relevant health professionals, an electronic medical assessment form for GPs to use in the referral process and a joint GP and school clinical assessment referral pathway. In the rural setting, the initiative saw Divisions of General Practice and schools work together to build better networks of care for students.
